Gallant & Associate’s Intensive Case Management (ICM) saves lives!
ICM is based on a classic social work model. G&A helps ensure continued success in recovery by providing structure and accountability. Our program is designed to complement and support recovery skills learned during inpatient treatment. The first 72 hours after leaving inpatient treatment is a difficult time – presenting the highest potential for relapse. As a team, our staff, along with treatment center professionals, family and the client will craft a Comprehensive Treatment Plan (CTP) ensuring comprehensive care and guidance. An individualized CTP is based on client issues, progress in treatment, and continuing care recommendations made by the treatment center. We continue to support each client by connecting them with specific recovery resources in their home communities and addressing real life issues as they arise. The CTP is our “roadmap” of support through the first 90 days.
We do not allow people to “fall through the cracks”. Our Case Managers accompany the client home and assist with the re-integration process. We help clients get to their first meetings and therapy appointments as well as look at the “people, places and things” that may lead to relapse. Our trained staff can remain with the client 24/7 for the first few days. We can facilitate “family sessions” or “return to work meetings”. We believe in the “Family System” approach and will assist the client in dealing with over-anxious loved ones by helping to set boundaries and limits. Gallant & Associate’s Intensive Case Management reduces the families’ need to play policeman. They can rest assured knowing that their loved one is on the right path.
We only work with a few clients at a time. We are fully committed to our clients and are available 24/7. Our clients understand that if a crisis situation arises in the middle of the night, we respond immediately and manage the situation. We engage our clients daily through face to face meetings and phone contacts. Our goal is to help each client understand that, “You are not alone.”
